Executive Contact Sheet Form: 2013-2014 version available here
The new form for the 2013-2014 academic year is now posted below! All SSMU clubs, services and publications must fill out this form at the beginning of every school year, by the deadline set by the Vice President (Clubs and Services) and publicized through the C&S Listserv. Failure to do so will lead to the group being deemed inactive. Only the names on this sheet will be able to book rooms, pick up cheques, retrieve or reset door codes and have access to other privileges available only to club and service executives. It is also a way of SSMU having contact information for each club so we can get in touch with you if needed.
*If you completed the 2012-2013 form but will not elect new executives until next year, don’t worry — that form will stay in use until the deadline for submission of the 2013-2014 form, by which point you will need to have elected and reported your new executive/collective members.
If at any point during the year you need to change information on this form, please contact the interest group coordinator at igc@ssmu.mcgill.ca
Tous les clubs, services et groupes de communication doivent remplir le formulaire qui suit au début de chaque année scolaire. Le groupe omettant de le remplir pourrait être considéré inactif. Seul les gens dont le nom sera mentionné sur le formulaire pourront réserver des locaux, percevoir les chèques, connaître et réinitialiser les codes des portes, et jouir des privilèges accordés aux membres de la direction des clubs et services seulement.
Si, au cours de l’année, vous devez apporter des modifications au formulaire, veuillez s’il vous plaît en informer le coordinateur de groupe d’intérêts : igc@ssmu.mcgill.ca.
