Insurance Form

If you are an active club via SSMU, you must fill out this form once each year. The data will be sent to our Club Insurance Coordinator (under the purview of the VP Clubs and Services).

If they are any concerns, the Coordinator will contact you.

You need insurance if you are doing anything outside of the University Centre!
